At the Association of Family Health Teams of Ontario —or just AFHTO for short—we’re on a mission to make primary care better for everyone in Ontario, both patients and providers. We’re a hub for collaboration in care, where evidence-based practice is shared and a community of innovators unites to transform the system.
We fight for fair, sustainable funding to ensure primary care teams have the resources needed to deliver top-tier care. Together with nearly 200 member organizations of almost 5,000 healthcare professionals, administrators and staff,
we care for 3.5 million Ontarians and growing.

Primary care teams are usually led by family physicians or nurse practitioners working alongside dietitians, social workers and other dedicated healthcare professionals, working together to care for you, the whole you.
While every team looks a little different, they all share a common goal: care through collaboration.
These teams are essential to making Ontario’s Your Health Plan a success, cut wait times and make healthcare more accessible for everyone.
